Care designed around you

All too often the focus is on the medical aspects of a condition, but what about the other aspects that are just as important?  We understand that a life-limiting illness affects you in lots of ways: physically, psychologically, socially, spiritually and emotionally. We provide services that see you as a whole person, and see what’s important to you, what you would like to achieve and your individual experiences. We can then work alongside you and your family or carers to find the best support for you.  Our team work closely together with your priorities in mind. We have a number of different professionals with a variety of skills and experiences to meet your individual needs. We offer a range of ways to access our services, depending on what’s most suited to you.

Living Well programme

Our Living Well programme offers a personalised, holistic approach to your wellbeing, through therapies, groups and activities.
