Our specialist Social Care team work with patients, carers and families to manage practical and emotional issues related to a life-limiting illness. If you’re a patient, the team can help you navigate any disadvantages you’re feeling as a result of your illness and make sure you’re not feeling socially isolated. You may find that accessing services in the community becomes harder, which means you’re relying on your support networks more. Our goal is to help you make choices that mean you can live as independently as possible. When someone has a life-limiting illness, the impact is often felt by everyone in the family, so our team also support carers and loved ones.
