Frequently asked questions
The programme is open to any adult diagnosed with a life-limiting condition, who is registered with a GP in the Milton Keynes area. You do not need to be under the care of other Hospice services.
You can be referred to the programme by any registered health professional. This includes GPs, district nurses, specialist nurses and therapists. They will need to complete a referral form.
At the moment, we’re unable to accept self-referrals, so you can’t directly ask to join the programme. However, we have a number of drop-in sessions that are open to anyone, without a referral. If after meeting you we feel that we can support you further, we can then refer you into the programme ourselves.
