Living Well programme

Our Living Well programme aims to help you live life the way you want, as best possible.

Helping you live well

Our Living Well programme is delivered by our Therapeutic & Wellbeing team. It aims to help you live life the way you want, as best possible. We support patients and their loved ones in managing and adapting to the challenges of living with a life-limiting condition through a range of activities and therapies. These help build confidence, promote independence and better wellbeing, and ensure everyone maintains a good quality of life for as long as possible.

Frequently asked questions

The programme is open to any adult diagnosed with a life-limiting condition, who is registered with a GP in the Milton Keynes area. You do not need to be under the care of other Hospice services.

You can be referred to the programme by any registered health professional. This includes GPs, district nurses, specialist nurses and therapists. They will need to complete a referral form.

At the moment, we’re unable to accept self-referrals, so you can’t directly ask to join the programme. However, we have a number of drop-in sessions that are open to anyone, without a referral. If after meeting you we feel that we can support you further, we can then refer you into the programme ourselves.

You’ll meet with one of our therapists, who will talk to you about what matters most to you. Together, you’ll identify what we can do to support you with reaching your goals.
We recommend that you attend each session to get the most from them. However, we understand this isn’t always achievable. We work flexibly with you to ensure that you benefit from the sessions.
Absolutely. If you feel more comfortable coming along with someone you know, you are more than welcome to bring them. There are certain situations where this isn’t recommended, but we’re happy to talk to you about this if needed.
Some of the sessions and classes are more interactive and more social than others, which is something we can discuss at your first appointment. We won’t force you into doing something you don’t want to do, but sometimes it’s good to try something you haven’t done before or wouldn’t normally try. We do have one-to-one appointments with members of the team if this is identified as being a better option for you.
The programme runs on a 6–8 week basis. After this time, we’ll talk with you about your goals and plan your next steps. This might involve attending other Hospice groups or services, or signposting you to other services in the community.
