Accessing our care

We care for adult patients, aged 18 or over, who have been diagnosed with a life-limiting illness. Our care can be accessed through a referral from a healthcare professional, or a self-referral completed by a patient or their carer.

Alternatively you can call our Palliative Care Hub directly for advice, as long as you or your loved one are registered with a GP in Milton Keynes. 

If you’re a healthcare professional wanting to refer a patient to our services, you’re in the right place. Please choose a service from the options below, complete the relevant form and send this to willen.hospice@nhs.net.

If we accept the referral, we’ll be in touch with your patient directly. If your patient hasn’t heard from us, or your referral is urgent, please call us on 01908 663636 and ask to speak to the relevant service.

Hospice care

You can refer a patient to us for specialist complex symptom control, including pain relief and management of distressing symptoms that are a result of their illness or treatment. They can be cared for either in our In-Patient Unit or by our Willen at Home team. We aim to contact your patient within 48 hours of referral.

Lymphoedema service

You can refer a patient to our Lymphoedema service if they have developed a swelling either secondary to cancer or its treatments, or as part of a palliative diagnosis. We also accept patients with primary lymphoedema who have a confirmed diagnosis from St George’s University Hospital NHS Trust Lymphoedema/Dermatology Service.

Therapeutic and Wellbeing service

You can refer a patient to our Living Well programme if they require support to manage their symptoms and achieve their individual goals earlier on in their palliative journey. We aim to contact your patient within five days of referral, or we will contact you directly if the referral is declined. You can also signpost your patient to our drop-in sessions, which do not require a referral
